Transitional Kindergarten to 8th Grade Registration

The Lennox School District offers various programs that include a Readiness Program from birth to age five, a Preschool Program, five elementary schools, and one middle school (grades sixth through eighth). We also offer a comprehensive After-School Program at each of our schools for eligible students. Additionally, at each of our schools there is an active parent program where parents are encouraged to become involved and volunteer to support the school programs.
 
We are proud to be a District of Choice and are accepting students from all surrounding areas to attend our schools.

Registration

To register your student you will need the documentation below.

To assist you in the process we made some resources available to you here.

 

#1 Proof of Residency BP 5111.1, EC 48200, EC48, EC 4820L)

Two of the following verifications have been accepted as proof of Lennox District School Residency (EC 48204.1, 48200,4820) Reasonable evidence of residency for a pupil living with his or parent or legal guardian shall be stablished by documentation showing the name and address of the parent or legal guardian with the school district, but not limited, any of the following documentation:

 

  • Property tax payment receipts
  • Rental property contract, lease or, or payment receipts
  • Utility service contract, statement, or payment receipts
  • Pay stubs
  • Voter registration
  • Correspondence from a government agency
  • Declaration of residency executed by the parent or legal guardian of a pupil

 

#2 Proof of Age (BP 5111/AR 5111)

Evidence of the child's age may include: (Education Code 48002)
  • A certified copy of a brith certificate or a statement by the local registrar or country recorder certifying the date of brith
  • A duly attested baptism certificate
  • A passport
  • When none of the above documents is obtainable, an affidavit of the parent/guardian
  • Other means prescribed by the Board.

#3 Health Screening or CDHP Exam (Health and Safety Code 124040, 124085, BP 5141.32, BP 514)

#4 Immunization Records (Health and Safety Code 124040, 124085, BP 514.3)

Entering TK/Kindergarten

Students entering school must show proof of the following immunizations:

Immunization

Dosage

Diphtheria, Pertussis, and Tetanus (DPT)

Five (5) doses

Polio

Four (4) doses

Measles, Mumps, and Rubella (MMR)

Two (2) doses

Hepatitis B

Three (3) doses

Varicella (chickenpox)

One (1) dose

 

Entering 7th Grade

Students entering 7th grade must show proof of the following immunizations:

Immunizations

Dosage

Tetanus, reduced Diphtheria, and acellular Pertussis (TDAP)

One (1) doses

Measles, Mumps, and Rubella (MMR)

Two (2) doses

 

Senate Bill (SB) 277 - Exemptions

SENATE BILL (SB) 277
Effective January 1, 2016, parents or guardians of students in any school or child-care facility, whether public or private, will no longer be allowed to submit a personal beliefs exemption to a currently-required vaccine.

Students will no longer be required to have immunizations for entry if they attend:

A home-based private school or
An independent study program with no classroom-based instruction.
However, parents or guardians must continue to provide immunization records for these students to their schools, and schools must continue to maintain and report records of immunizations that have been received for these students. 

Students who have an individualized education program (IEP) should continue to receive all necessary services identified in their IEP regardless of their vaccination status.

#5 Oral Health Assessment (EC 49452.8)

 

California law now requires that your child have an oral health assessment in kindergarten or first grade, whichever is his or her first year of public school.
