IMPORTANT: ENROLLMENT FORMS NOW ONLINE
Dear Parents and Guardians,
We are excited to inform you that the Lennox School District has transitioned from paper
forms to PowerSchool Enrollment online. Starting this school year, all required beginning-ofthe-
year forms will be completed electronically.
These forms include:
- Annual Notification of Parent or Guardian Handbook
- Emergency Contacts
- Student Residency Questionnaire (McKinney-Vento Act Form)
- Technology Use Agreement
- Google App Permission
- Photo/Video Release
- Walking Home from School Permission
- Student Medical Information
You will soon receive a separate email with your child’s name and a unique 15-character snap
code. This snapcode will allow you to access your child’s forms through the links below:

If you need help completing the forms, your child’s school will have computers available, and staff will be happy to assist you.
